Description: 

The NYC agency is looking to hire Social Work Supervisors, Case Associates and Case Assistants for their Manhattan and Bronx locations. The employer will be conducting one on one interviews onsite at the Washington Heights Workforce1 Center on Thursday June 14th from 11:00 am to 1:30 pm.

SOCIAL WORK SUPERVISOR

Salary: 58k to 62k

JOB DESCRIPTION OVERVIEW: The Social Work Supervisor has clinical and administrative responsibility for an apartment program unit, under the supervision and direction of the program director. The Social Work Supervisor is responsible for ensuring the provision of services for residents, oversight of administrative processes, and supervision of apartment program staff.

KEY ESSENTIAL FUNCTIONS (List the functions that are essential to achieve the goals):

Clinical:

• Provide weekly individual supervision of all assigned program staff.

• Responsible for the provision of day-to-day services to program residents.

• Provide direct service and/or coverage to residents consistent with restorative services and service plan goals.

• Plan and participate in program activities.

• Facilitate staff briefings, team meetings and case conferences.

• Facilitate Consumer/Occupant Meetings.

• Review and co-sign all assessments, individualized service plans and progress notes.

• Complete required documentation and assist supervisees in completing documentation consistent with Medicaid, OMH and JBFCS JOB REQUIREMENTS and guidelines.

• Facilitate intakes and discharges.

• Develop and maintain linkages with community service providers.

• Conduct Utilization Reviews.

Administrative:

• Participate in interviewing and hiring program staff.

• Address staff performance issues in accordance with JBFCS Policy & Procedure.

• Conduct staff training & development, as required, and conduct performance appraisals.

• Schedule and monitor staff attendance and staff scheduling to ensure proper coverage.

• Conduct regular apartment visits and walkthroughs.

• Maintain relationships with landlords, handle leases, resolve apartment-related and facility issues.

• Conduct Quality Assurance / documented chart review and audit.

• Coordinate and oversee admission and discharge processes in accordance with all relevant regulations.

• Prepare and submit monthly reporting and billing.

• Submit monthly internal JBFCS reports.

• Monitor program for continued improvement.

• Participate in all meetings, as assigned.

• Conduct outreach; maintain contact with referral sources; maintain census.

• Work collaboratively as part of a service delivery team. Crisis:

• Provide and supervise crisis intervention services.

• Ensure Crisis Intervention Event Reporting and follow up.

• Provide on call administrative and crisis evening and weekend duties.

EDUCATIONAL / TRAINING REQUIRED (List all that is required to achieve this position):

Masters in Social Work (MSW) Degree and NYS licensure as a LMSW are required.

EXPERIENCE REQUIRED / LANGUAGE PREFERENCE (List what the desire to reach goals): A minimum of 2 years of experience with working with adults with mental illness is required. Supervisory experience is preferred. Excellent case work, administrative, managerial and computer skills are required

CASE ASSOCIATE

Salary: $18.60 - $20.00 per hour

JOB DESCRIPTION OVERVIEW: The Case Associate functions as a case planner and is responsible for supplying appropriate interventions to ensure client care. Working within the client’s community, develops action plans to effectively identify and utilize existing resources and establish new resources for client care.

RESPONSIBILITIES include but are not limited to

• Routine community contacts on behalf of social worker, e.g. school reports, medical reports.

• Locates and makes use of appropriate community resources for clients.

• Evaluates actual living conditions of clients through home visits.

• May interpret agency function to client and demonstrate this in terms of concrete help.

• Provides structure to the client, evaluates and assess usage of independent living skills and provides guidance in these areas to the client.

• May run groups, under the supervision of the social worker.

• In concert with the Sr. Case Planner, evaluates clients for the purpose of beginning, monitoring and following up on individualized treatment programs including child safety.

• Individualizes and understands the needs of the individual client, the family and its support network in relation to each other.

• Identifies appropriate treatment or service providers and facilitates client referral to internal and external and/or community linkage programs.

• Performs individual/group counseling; long or short term depending on client need.

• Conduct Home Visits in which the needs of the family and child safety are assessed. Identify help for clients in obtaining needed services. Helping clients and families maintain medical, financial and social resources would be examples of such services.

• Provides case management to clients.

• Identifies and provides access to specific appropriate concrete services when necessary or requested and are in the best interest of the client.

• Performs community outreach and establishes linkages with other groups, individuals or organizations which may be helpful to our clients.

• Provides all necessary notes, progress notes, Risk Assessments and other paperwork as required.

ADDITIONAL RESPONSIBILITIES-:

Establish a liaison with school officials to develop a stronger home/school link and secure educational records.

JOB REQUIREMENTS BA Degree, with some experience in a community or social service group, or at least five years’ experience as a Case Assistant in a community or social service group, etc.

CASE ASSISTANT

Salary: $18.60 per hour

JOB DESCRIPTION

Case Assistants in the AMI Apartment Programs provide apartment residents with training and assistance in basic living skills and provide other supportive services to help residents achieve and maintain their ability to live independently and realize their community integration goals.

RESPONSIBILITIES include but are not limited to:

• Provide in-home services to residents within the scope of program regulations and mission

• Carry primary responsibility for a caseload under supervision

• Provide a range of direct services, including active, hands-on skills training and supportive counseling, targeted to help residents reach wellness and community integration goals

• Provide instruction in general safety and security to all residents

• Conduct regular safety and maintenance checks of the apartments; assist in resolving facility-related problems

• Conduct apartment meetings, helping roommates live together successfully

• Ensure that residents understand the rules and occupancy requirements of the program and assist them in meeting them

• Complete all required documentation, such as admission package, assessments, service plans and reviews, progress notes, medication logs, walk-through and safety logs in a comprehensive and timely manner

• Prepare and submit monthly billing documentation, if applicable

• Assist residents in discharge planning

• Provide linkage and case management services

• Maintain contact with primary treatment providers or other service providers to coordinate service plans, attending case conferences, as needed

• Involve family members in service planning, as indicated

• Escort and advocate for residents (court, school, hospital, benefits offices)

• Provide crisis management and intervention

• Attend staff meetings and trainings

• Participate in resident community meetings and group services

• Other duties as assigned

JOB REQUIREMENTS Minimum educational level of High School. Minimum of two years related work experience, preferably working with adults living with mental illness or in a residential treatment program.
